| Crown, bridges and onlays | We will replace or repair your crown, bridge or onlay with the same type of restoration at no extra charge
to you within 3 years. If your restoration will require a replacement with more expensive one within 36 months, you will only pay the difference. |

Sometimes root canal or gum treatments may become necessary on the same tooth, these are not covered by the warranty, all extra procedure will be completed at additional cost. |

| Dentures and partials | We will repair dentures and partials during a period of 3 years from the placement date,
if a tooth chips, breaks or flange breaks under normal use.
This warranty will not apply if you or anyone else breaks your denture/partial as a result of an accident (examples - car accident, dropping your denture in the sink, floor) If necessary, we will reline your denture once within the warranty period at no extra fees. |
Patients wearing full upper and lower dentures must be seen once every 12 months in OUR office.
Patients who have natural teeth remaining must be seen at the prescribed recare appointments in our office, most commonly every 180 days or less, that includes exam, cleaning and x-rays. You must follow prescribed oral hygiene instructions that are reviewed at every recare appointment. |
